  • Copy of child's Birth Certificate

  • Copy of a parents Catholic Baptismal Certificate

  • Parents are asked to submit a letter as to their path going forward in raising the child in the Catholic Faith.  Example; Mass you will be participating in, faith practices in your home, sacraments for both the child and parents, ways the Parish can assist you in both community and raising your child in the Catholic Faith.  Please email the letter to sacraments@ascensionparish.ca . Please address the letter to our Pastor Fr. John Luxbacher.

  • Godparents - A child may have only one or two Godparents at Baptism. If you choose to have two Godparents, you must have one male and one female.

    • The Godparent(s) must be fully initiated, practicing Catholics, having received the Sacraments of Baptism, First Communion, First Reconciliation, and Confirmation.

    • The Godparent(s) must be at least 16 years of age or older.
